MEDICAL WAIVER

Completion and submission of the medical waiver below is mandatory for all participants. Final authorization to dive rests exclusively with the lead Divemaster, who reserves the right to deny participation based on medical assessment and safety considerations.

Pre-Arrival Waiver Review: We strongly recommend reviewing the medical requirements prior to your visit. If you answer “yes” to any medical conditions or have health-related questions:

1. Complete the waiver in full.

2. Email the completed form to us prior to your tour date.

3. Our Divemaster will review your information to ensure you are cleared to dive.
